Notice of privacy practices

Texas Pathology Group, LLCEffective August 1, 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who this notice covers

This is a joint notice for Texas Pathology Group, LLC and the physicians and staff of its associated practice — Texas Pathology Consultants, P.A. — at 11838 Medpark Drive, Suite 103, Burleson, TX 76028. When your physician sends a specimen to us, or one of our pathologists examines your case at a hospital we serve, we create and hold medical records about you.

Our responsibilities

  • We are required by law to maintain the privacy and security of your protected health information (45 C.F.R. Parts 160 and 164; Tex. Health & Safety Code ch. 181).
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information (45 C.F.R. §§ 164.400–164.414).
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing. You may change your mind at any time; tell us in writing and we will stop, except to the extent we have already acted on your permission.

How we may use and disclose your information

Treatment. We report your diagnosis to the physician who ordered the test and to others treating you, and we may consult other physicians — for example, a subspecialty pathologist reviewing a difficult case.

Payment. We use and disclose your information to bill and collect payment for our services from you, your health plan, or others responsible — including the billing staff of Texas Pathology Services acting on our behalf.

Health care operations. We use your information to run the practice: quality assurance and peer review, accreditation and inspection of our laboratories, training, and business management. We may share it with business associates — such as software, courier and reference-laboratory vendors — who are bound by written agreements to protect it (45 C.F.R. §§ 164.502(e), 164.506).

As required or permitted by law. We may disclose information: for public-health reporting to authorities such as the Texas Department of State Health Services, including required disease and cancer-registry reporting; to health-oversight agencies for audits and inspections; in response to court orders, subpoenas and other lawful process; to law enforcement in limited circumstances; to coroners, medical examiners and funeral directors; for organ and tissue donation; for workers’ compensation claims; to avert a serious threat to health or safety; and for research approved under legally required protections or using de-identified information.

Uses that require your written authorization. We will not use your information for marketing, sell it, or share psychotherapy notes (which we do not hold) without your written authorization. We do not use patient information for fundraising.

Your rights

  • Get a copy of your record. Ask us in writing; we will provide a copy, usually within 30 days, and may charge a reasonable cost-based fee. Laboratory results are also available from the physician who ordered them, who is the right person to explain what they mean.
  • Ask us to correct your record. If you believe information is incorrect or incomplete, ask us in writing. We may decline, and will tell you why in writing within 60 days (45 C.F.R. § 164.526).
  • Get a list of disclosures. You may ask for an accounting of certain disclosures we have made in the six years before your request — one free per year (45 C.F.R. § 164.528).
  • Ask us to limit what we share. You may request restrictions; we are not required to agree, except that if you pay for a service in full out of pocket, we must not share that information with your health plan for payment or operations if you ask us not to (45 C.F.R. § 164.522).
  • Ask for confidential communications. You may ask us to contact you in a specific way or at a specific address; we will accommodate reasonable requests.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. A person with medical power of attorney or a legal guardian may exercise your rights on your behalf; we will verify their authority.
  • File a complaint. Complain to us using the contact below, or to the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, DC 20201 · 1-877-696-6775 · hhs.gov/ocr. We will not retaliate against you for filing a complaint.

Changes to this notice

We can change this notice, and the changes will apply to information we already hold. The current notice, with its effective date, is always available on this page and on paper at our office.
